Expanded Benefits - Other Services
Assisted Living Facility/Adult Family Care Home/Group Home Bed Hold Days - No prior authorization is needed. A bed will be held up to 31 days per month at the members home facility when receiving care at another facility.
Digital Library Membership with E-reader - Members receive an e-reader and a membership to a digital library with unlimited downloads that include hundreds of thousands of books available in the catalog. Prior authorization is required.
Gym Membership - Members may receive up to $50 per month for an annual membership at a health club. Prior authorization is required.
Home Delivered Meals - Members may receive unlimited services with prior authorization per their plan of care.
Housing Assistance - Members may receive up to $1,000 lifetime in housing assistance (such as rent, utilities, or groceries). Prior authorization is required.
Meal Stipend - Members may receive unlimited services with prior authorization per their plan of care. This service is available for long distance medical appointment daytrips and includes up to twenty dollars ($20.00) per trip in excess of one hundred (100) miles for the member and an escort.
Nursing Facility to Community Transition Assistance - Members may receive up to $5,000 per transition when transitioning from a nursing facility to a community setting. Prior authorization is required.
Waived Copayments - Co-payments are waived for the following services: Chiropractor services, Community Behavioral Health services, Home Health services, Hospital Outpatient services, Federally Qualified Health Center visits, Independent Laboratory services, Non-emergency Transportation services per each one-way trip, Nurse Practitioner services, Optometrist services, Physician and Physician Assistant services, Podiatrist services, Portable X-Ray services, Rural Health Clinic visits, and use of hospital emergency department for non-emergency services.
