Expanded Benefits - Professional and Home Care Services
Doula Services – Members may receive unlimited services with prior authorization per their plan of care. Services include home visit for prenatal monitoring and assessment, postnatal assessment and follow-up care, and newborn care and assessment.
Financial Literacy – Members may receive unlimited services with prior authorization. Services include life coaching for money management/budgeting, access to a financial vendor for investment guidance, financial counseling, financial consults, advice, and tax consultation. Members also receive a 25% discount on tax preparation and credit repair.
Guardianship Benefit - Members may receive up to $1,000 to obtain guardianship, and up to $250/year to maintain guardianship. Prior authorization is required.
Home Health/Nursing Aide - Members may receive unlimited services with prior authorization per their plan of care. Services include unlimited visits by a home health aide or certified nurse assistant (per hour and per visit), unlimited nursing care in the home provided by a registered nurse or licensed practical nurse (per hour and per diem), and unlimited personal care services (per fifteen (15) minute units and per diem).
Home Visit - Clinical Social Worker - Members may receive unlimited services with prior authorization per their plan of care. Includes the services of a clinical social worker in home health or hospice settings.
Homemaker Services - Members may receive unlimited services with prior authorization per their plan of care. This service includes help with general household activities, like meal preparation and routine home chores. Available in fifteen (15) minute increments.
Nutritional Counseling - Members may receive unlimited services with prior authorization per their plan of care. This service includes nutrition class, initial and subsequent individual medical nutrition, group medical nutrition, and individual and group medical nutrition therapy after a change in diagnosis, medical condition, or treatment regimen.
Swimming Lessons (Drowning Prevention) - Members age 18-20 may receive unlimited services with prior authorization per their plan of care.
Tailored Caregiver Assessment and Referral (TCare) - Members may receive Home Care Training sessions for family or non-family. Available services include one assessment session and three quarterly follow-ups with prior authorization.
