Familial Dysautonomia Waiver
Purpose
The purpose of the Medicaid Familial Dysautonomia (FD) Waiver is to provide home and community-based supports and services to eligible persons with Familial Dysautonomia living in their own homes or family homes. The FD Waiver is designed to promote, maintain and restore the health of eligible recipients with Familial Dysautonomia and to minimize the effects of illness and disabilities through the provision of needed supports and services in order to delay or prevent hospital placement or institutionalization.
Eligibility Criteria
To qualify for the FD Waiver the applicant must:
- Have a documented diagnosis of Familial Dysautonomia
- Be three years old or older
- Be assessed as having an Inpatient Hospital Level of Care (LOC)
Covered Services
Waiver services include:
- Behavioral Services
- Consumable Medical Supplies
- Dental Services
- Durable Medical Equipment
- Non-Residential Support Services (NRSS)
- Respite Services, and
- Support Coordination
