If your complaint is regarding Medicaid eligibility or the application process, please contact the Department of Children and Families ACCESS Program.

If your complaint is against a doctor, and involves a quality of care issue, please submit a complaint to the Florida Department of Health using the Practitioner Complaint Form[ application/pdf ].

If you think a health care provider or a Medicaid recipient has intentionally misused someone’s Medicaid coverage to receive services they should not have received, please contact the fraud and abuse hotline at 1-888-419-3456, or submit a Medicaid Fraud and Abuse Complaint using the online Fraud Complaint Form.

If you think a health care provider is committing Medicaid billing fraud, please contact the fraud and abuse hotline at 1-888-419-3456, or submit a Medicaid Fraud and Abuse Complaint using the online Fraud Complaint Form.

If you think a person gave fraudulent information about his/her income, assets or living circumstances to qualify for Medicaid, please contact the Florida Department of Children and Families, Office of Public Benefits Integrity at Public Benefits Integrity | Florida DCF (myflfamilies.com).

If you need to request copies of your medical records, please contact your health care provider directly.  If you believe that the provider is not complying with your request, you may file a complaint with the Department of Health.  For all other requests, contact the Agency's Public Records Office.
