Outcome and Assessment Information Set (OASIS)
Florida home health agencies that are Medicare-certified or Medicaid-enrolled are required to electronically transmit to the state database in accordance with the federal reporting regulation (effective July 19, 1999). Florida HHAs should be using CMS-issued branch identifiers in M0016 instead of the prior Florida requirement to enter the state license number (without the preceding 'HHA' designation).
The OASIS Helpdesk has an e-mail alert system to notify registered providers or interested parties of "late-breaking" OASIS information. Complete as much information using the registration link and check OASIS for Provider Type and Opt-In to receive emails or Opt-Out if you wish to stop receiving emails.

- OASIS Exemption Attestation (OASIS submission is required for HHAs participating in the Medicare and/or Medicaid programs under 42 CFR §484.20 (G320), a federal condition of participation. OASIS does not apply to HHAs meeting certain exemption requirements. In an effort to facilitate submission compliance monitoring, the exemption attestation needs to be submitted annually or updated upon changes in exemption status.)

OASIS Test Accounts:
HHAs use the OASIS Validation Utility Tool (VUT), available on the iQIES homepage at iQIES Validation Utility Tool (VUT) to submit test XML files to provide evidence of their ability to submit patient assessments to CMS.
The OASIS VUT is a software utility used to validate OASIS submission files in XML format. A login to iQIES is not required to access the iQIES OASIS VUT. Upon submission, users will have the option to download a validation report displaying their submission results to be provided to the surveyor as part of the pre-certification process.
Medicare Beneficiary Identifier (MBI) Format Information:
Print the confirmation page stating the submission was accepted for your records. A sample is attached.
Note: It is recommended that users access iQIES using Chrome or an Internet browser alternative to Internet Explorer.
